You may make an appointment for yourself or a loved one in three ways:

2. Send an email to office@elderconsult.com.
3. Fill out appointment forms below and fax them to us at 650-357-8811.

Once the appointment has been set up, there are several documents we need before we can begin working with the patient. Typically, we email, mail or fax these to the patient in advance of an appointment, but they can also be downloaded. They are:

	Patient Information Sheet This is a one-page summary of essential contact information about the patient which needs to be completed and returned to us. You can download it here.
	Professional Services Agreement This is the legal agreement specifying what services we will provide, and details your rights and responsibilities as a patient. This form needs to be signed and dated. You can download it here.
	Authorization to Release Medical Information This form allows us to legally exchange information with other healthcare providers. It needs to be signed and dated. You can download it here.
	Geriatric Assessment Form This will provide more information about the concerns that are to be addressed. The examination will be less time-consuming and easier for the patient if the questionnaire is completed prior to the appointment. You may be instructed to input this from the web portal or fax it back. You can download it here.

In addition, new patients also receive a welcome letter, which sets out additional information about our services, coordination of care, and so on.

The consultation may be performed in the office at 533 Airport Blvd., Ste. 400 Burlingame, CA 94010, in the patient’s home or in a facility, nursing home or hospital. If mobility or willingness to travel to a doctor’s office is an issue, we are glad to make house calls.

Payment
ElderConsult is a private-pay service. Payment is accepted by check or credit card in advance of the consultation or at time of service.
